Learn how to fill out and submit a form

Before viewing the form, you will be greeted with a welcome message that displays the form's name.

If you are accessing this form while you are signed into a Script account, you will not be required to fill out your information before starting your submission.

If you are accessing this form while not signed into a Script account, you will be asked for the following information:

First Name

Last Name

Email

Electronic Consent


Once you have opened the form, there are a few important things to be aware of.

You might see a variety of ways that you are able to input your data, such as single lines of text, larger fields for multiple lines of text, dropdown menus, signature fields, date fields, multiple choice options, and checkboxes.

If you look above the first page of the form in the right corner, you'll notice that you can download a copy of the form, if it's a paper form, by clicking the arrow pointing down.

To adjust your view of the form, click the square of arrows next to the download button.

All fields that require your input will be presented in blue or white.

Required fields will be marked with a red asterisk.

Fields that appear gray are assigned to another user or they are formula or autofill fields that cannot be edited.

If additional instructions are supplied for a field, read the message by clicking the information button that appears as a circled lowercase "i".

In the top right corner of your screen, you will see a blue Save For Later button. You may use this option anytime you aren't ready to submit the form yet, but would like to save the data you've already entered on the form. When you click Save For Later, a green checkmark will appear to notify you that your form has been saved, and you will receive an email with a link that will take you back to your saved work at any time.

In that same corner, you will also see a purple Submit button that, once clicked, will submit your form to the next step in the workflow. You might have additional steps that are assigned to you that will appear once you submit a form, but that will depend on the workflow.

Once you've submitted your form and there are no additional steps for you to complete at this time, you will see a "Submission Complete!" message, along with the ability to download your submission. You will also receive an email confirming your completion of this form with another link to download your submission directly from the email.

You are now ready to start completing forms in Script!
